This communication is from a debt collector. This is an attempt to collect a debt and any information obtained will be used for that purpose.

I authorize CBD to contact me at the phone number provided. CBD may leave a message on the voicemail for this phone number disclosing the call is from a debt collector at Credit Bureau Data, Inc.

RISKS OF USING EMAIL

Communication by email has a number of risks which include, but are not limited to, the following:

  • Email can be circulated, forwarded and stored in paper and electronic files.
  • Backup copies of email may exist even after the sender or the recipient has deleted his/her copy.
  • Email can be received by unintended recipients.
  • Email can be intercepted, altered, forwarded or used without authorization or detection.
  • Email senders can easily type in the wrong email address.
  • Email can be used to introduce viruses in computer systems.

CONDITIONS FOR THE USE OF EMAIL

Credit Bureau Data, Inc. (“CBD”) will use reasonable means to protect the security and confidentiality of email information sent and received. However, because of the risks outlined above, CBD cannot guarantee the security and confidentiality of email communication, and will not be liable for improper disclosure of confidential information that is not caused by CBD’s intentional misconduct. Thus, the consumer must consent to the use of email for confidential information. Consent to the use of email includes agreement with the following conditions:

  1. Consent applies to all collection accounts listed at CBD as of the date of the consumer signs this consent form and is valid until CBD receives a written request revoking consent to email.
  2. Images of the emails may be stored by CBD as part of the documentation regarding a collection account(s).
  3. CBD may forward your email internally to CBD staff members as necessary for appropriate handling. CBD will not forward emails to independent third parties, except for the original creditor or agent of the original creditor, except as authorized or required by law.
  4. CBD is not liable for breaches of confidentiality caused by the consumer or any third party.
  5. CBD will make every attempt to read and process all emails within one business day.
  6. CBD will send a standard email message and will attach additional documents potentially containing confidential information as a PDF.
  7. The consumer is responsible for informing CBD of any types of information the consumer does not want to be sent by email.
  8. The consumer is responsible for protecting his/her password or other means of access to email.
  9. Due to licensing requirements, CBD may not be able to send emails to residents of some states or countries.
  10. Consumers have the right to withdraw consent without penalty by contacting CBD at [email protected], calling (608) 791-2143 or (888) 378-2375, or writing to CBD at PO Box 2288, La Crosse, WI 54602-2288.

INSTRUCTIONS

To communicate by email, the consumer shall:

  1. Do not use your employer’s computer or employer-provided email account to connect with CBD. There is no expectation of privacy of emails sent or received on an employer’s computer or through an employer provided email account.
  2. Include your name, phone number, and address in the corresponding fields and, if known, the CBD account number.
  3. Review the email to make sure it is clear and that all relevant information is provided before sending it to CBD.
  4. Send CBD an email confirming receipt of a message from CBD after you have received and read the email message.
  5. If your email requires a response from CBD, and you have not heard back from CBD within three (3) business days, call CBD to follow up and determine if CBD has received your email.
  6. Take precautions to protect confidentiality of email such as safeguarding your computer password(s) and using screen savers.
  7. Inform CBD of changes in your email address.
  8. Requests to withdraw consent must be submitted to CBD only by email to [email protected] or written communication to CBD at PO Box 2288, La Crosse WI 54062-2288.

EMAILS FROM CBD

Emails from CBD will be encrypted. The body of the email will contain only a standard email message. Requested information will be attached to the email. The entire email will be sent via a secure server.

  1. CBD will send emails through a secure email portal which sends the consumer a generic email message containing verbiage such as, “You have mail at [name of secure portal]. To read your mail, click the link below to be directed to a secure server.” When clicking on the link provided, the consumer is directed to a site where he/she must sign in with a previously assigned username and password.

ACKNOWLEDGEMENT AND AGREEMENT

I acknowledge that I have read and fully understand this consent form. I understand the risks associated with the communication of email between CBD and me, and consent to the conditions herein. I understand that CBD cannot guarantee the security and confidentiality of email communication. CBD will not be responsible for messages that are not received or delivered due to technical failure, or for disclosure of confidential information unless caused by intentional misconduct by CBD. In addition, I agree to the instructions outlined herein as well as any other instructions that CBD may impose to communicate with me by email. I understand that either I or CBD may stop using email as a means of communication upon my written request. I have read and understand this form. I understand and agree with the information contained herein and give my consent for email communications to and from CBD to the above listed email address.
